  • Be provided with complete and current information concerning your diagnosis, treatment, and prognosis to you or your designee.
  • Be informed of the consent process for any and all treatments and procedures (with the exception of emergency medical care). The informed consent process includes an explanation of the treatment or procedure; any alternative treatments or procedures; the intent, and possible complications of the treatment or procedure; and the anticipated outcome.
  • Be explained the cost for testing and treatment and explanation of your bill.
  • The opportunity to submit any concerns in writing and to receive an appropriate response.
  • The opportunity, if you wish, to formulate advance directives and appoint someone else to make health care decisions to the extent permitted by law.
  • Be told of the fees not covered by OHIP and that you will have to pay.

It is important to:

  • Provide staff with accurate information about your health and your current treatment

  • Inform NewLife Fertility staff if your condition changes
  • Follow your prescribed treatment
  • Be considerate of staff and other patients
  • Attend your scheduled appointments, or inform staff if you need to change an appointment
  • Make treatment payments to NewLife Fertility, prior to commencing your treatment
  • Ensure you have a current referral to your fertility specialist and provide a copy of this to NewLife Fertility
  • Accept responsibility for the decisions, omissions you make about your treatment.
  • Respect clinic property and act in a safe and responsible way.
  • Understand and be responsible for all expenses not covered by OHIP or private insurance.
