Follow these simple steps to become NewLife Fertility patient:

Step 1: Download the referral form located below and provide the form to your medical professional.

Step 2: Inform your medical professional if you want a specific specialist at our centre or if you would like the first available appointment.

Step 3: Medical professional faxes your referral form to NewLife Fertility.

Step 4: You will receive a call from our New Patient Coordinator to book your appointment with your Specialist.

Once referred, you will be assigned a Specialist who will meet with you and make all decisions with you about your medical care.
